Description

The 11th Street Family Health Services Center is a nurse-managed health center providing comprehensive services to urban, medically underserved community members. The center, run by the College of Nursing and Health Professions in collaboration with the Family Practice and Counseling Network, provides community based care while educating health professions students. Low-income patients receive a comprehensive range of health services including physical exams, family planning, dental care, nutrition, physical fitness, behavioral health, and disease prevention services. More than half of the patients served by the center have Medicaid coverage, and an additional third are uninsured. The center offers health education outreach for patients, and a venue for the education of health professionals in culturally competent, community-based care.

Goal / Mission

The goal of the 11th Street Family Health Services Center is to provide healthcare to underserved, low-income community members.

Results / Accomplishments

The number of primary care visits provided by the 11th Street Family Health Center has grown every year, from 2,200 in 2003, to more than 10,000 in 2007. Visits to the center’s behavioral health services have also grown, from 210 in 2003, to 3,205 in 2007. Patients accessing dental services have expanded from 944 visits in 2005, to 6,772 visits in 2007. The center has also provided more than 2,767 health education and wellness program encounters. Chart reviews show improvements in pediatric immunization rates, and improved breast cancer screening.
